ESBLExtended-Spectrum Beta-Lactamase (ESBL)
Criteria: Reduced susceptibility to cephalosporins (cefotaxime MIC >1, ceftazidime MIC >1); synergy confirmed
Action: Report all penicillins and cephalosporins as Resistant regardless of MIC. Avoid aztreonam.
MRSAMethicillin-Resistant Staphylococcus aureus (MRSA)
Criteria: Oxacillin MIC >2 mg/L or mecA/mecC gene detected
Action: Report ALL beta-lactams as Resistant. Glycopeptide (vancomycin) or linezolid therapy.
CRECarbapenem-Resistant Enterobacterales (CRE)
Criteria: Meropenem or imipenem MIC >=4 mg/L; carbapenemase production suspected
Action: Activate infection control alert. Consider colistin/polymyxin B or ceftazidime-avibactam. Contact ID.
VREVancomycin-Resistant Enterococcus (VRE)
Criteria: Vancomycin MIC >=32 mg/L (vanA) or 8-16 mg/L (vanB)
Action: Activate isolation. Consider linezolid, daptomycin, or tigecycline. Notify infection control.
